Licensed Practical/Vocational Nurse (LPN/LVN) – Rehabilitation (PRN)

As a Licensed Practical/Vocational Nurse (LPN/LVN) who excels in this role, you will:

• Accurately perform patient assessments and identify patient needs
• Identify and initiate appropriate nursing interventions
• Provide care appropriate to the condition and age of the patient
• Perform timely and appropriate documentation related to medical necessity in the medical record
• Be responsible for completion and revision of the Interdisciplinary Care Plan for each patient
• Perform timely and accurate QI assessments

• Current state LPN license
• Basic Life Support certification required within 14 days of hire
• One year of experience preferred
