How You’ll ContributeAs a Licensed Practical/Vocational Nurse (LPN/LVN) who excels in this role, you will:
- Accurately perform patient assessments and identify patient needs
- Identify and initiate appropriate nursing interventions
- Provide care appropriate to the condition and age of the patient
- Perform timely and appropriate documentation related to medical necessity in the medical record
- Be responsible for completion and revision of the Interdisciplinary Care Plan for each patient
- Perform timely and accurate QI assessments
- Current state LPN license
- Basic Life Support certification required within 14 days of hire
- One year of experience preferred
