Heritage Senior Living - - Responsibilities:
Lead and assist with person-centered activities designed specifically for residents living with Alzheimer's disease and other forms of dementia;
Provide sensory stimulation, music, reminiscence, life skills, exercise, and therapeutic activities;
Build meaningful one-on-one relationships with residents through individualized visits and purposeful engagement;
Monitor residents for changes in mood, behavior, or condition and communicate concerns with nursing staff;
Document resident participation and responses to activities in Point of Care
