We’re hiring an LPN to provide resident care and clinical oversight at our Heritage Senior Living community in Muskego, WI. Working alongside our Director of Nursing, you’ll help our residents live comfortably and well. Heritage Senior Living provides exceptional senior care and services throughout Wisconsin. - Provide and oversee direct resident care according to each resident’s Individual Service Plan (ISP) and physician orders
- Carry out resident clinical needs management as directed by the DON, within the scope of the Nurse Practice Act, DHS 83, and Heritage Senior Living policies and procedures
- Apply your knowledge of nursing principles (geriatrics, senior living, and long‑term care) to each resident’s specific circumstances
- Help the DON prepare and maintain effective ISPs, and provide the information needed to keep them current
- Document detailed notes in residents’ electronic medical records that reflect the therapeutic plan and the specifics of the ISP
- Oversee the 24‑hour report, and make routine checks on any resident who is acutely ill or has a change in condition
- Oversee and support the management of medications, order reconciliation, and physician orders
- Valid State of Wisconsin LPN/LVN license, in good standing with the State Board of Regulation and Licensing
- Knowledge of nursing principles (geriatrics, senior living, and long‑term care) and professional standards of nursing practice, with the ability to apply them to resident‑specific circumstances
- Knowledge of Wisconsin state regulations for CBRFs and/or RCACs
- Basic computer skills, including EHR software
- Ability to speak, read, and write English
- Flexibility to handle variations in a daily schedule
- Ability to work 8‑ to 12‑hour shifts
