RN Case Manager / Assistant Director of Nursing
Muskego Health & Rehabilitation Center is seeking a compassionate, organized, and experienced Registered Nurse (RN) Case Manager / Assistant Director of Nursing (ADON) to join our skilled nursing team.
This is an excellent opportunity for an RN who enjoys combining resident-centered care coordination with clinical leadership
. The RN Case Manager / ADON will help coordinate resident care from admission through discharge while supporting the Director of Nursing with the daily operations of the nursing department.
Coordinate and monitor resident plans of care from admission through discharge.
Complete and monitor nursing assessments and identify changes in resident condition.
Communicate with residents and families regarding care needs, progress, and transitions of care.
Assist with discharge planning and coordination of services following discharge.
Monitor changes in resident condition and ensure appropriate follow-up and interventions.
Maintain accurate, timely, and complete clinical documentation
Identify individual social and emotional needs
Develop and maintain individualized care plans
Participate in the planning of the resident's admission, community re-entry or transfer to another facility
Assist the Director of Nursing with the daily operations of the nursing department.
Provide clinical leadership, guidance, mentoring, and support to nursing staff.
Monitor resident care and promote positive clinical outcomes
Participate in Quality Assurance and Performance Improvement (QAPI) activities.
Assist with survey preparation, regulatory readiness, and follow-up activities.
Promote a positive, collaborative, and professional work environment.
Participate in the weekend management rotation as assigned.
Current, active Registered Nurse (RN) license in the State of Wisconsin
.Experience in long-term care, skilled nursing, rehabilitation, or a related healthcare setting preferred
.Strong clinical assessment, organizational, communication, and leadership skills.
Knowledge of care planning, discharge planning, and regulatory requirements.
Ability to work effectively with residents, families, healthcare providers, and an interdisciplinary team.
Strong attention to detail and ability to prioritize multiple responsibilities.
Commitment to providing compassionate, high-quality, resident-centered care.
Previous supervisory or nursing leadership experience is preferred but not required.
