Schedule: Exempt | 40 hours per week
Monday – Friday | 8:00 AM – 4:30 PM
Location: Muskegon, Michigan
The Impact You’ll Make:As a Primary Care Physician, you will guide the medical care of PACE participants through comprehensive assessments, individualized care planning, and ongoing clinical leadership within the Interdisciplinary Team.
Key Responsibilities- Perform initial and ongoing comprehensive health assessments
- Lead clinical decision-making within the interdisciplinary care team
- Develop and oversee personalized care plans aligned with participant goals
- Partner with families to support informed, compassionate care decisions
- Manage chronic conditions with a proactive, preventative approach
- Collaborate with specialists and oversee care transitions, including skilled nursing stays
- Work closely with the Medical Director and Advanced Practice Providers to ensure coordinated, high-quality care

Skills, Knowledge & ExpertiseRequired:
- MD or DO from an accredited program
- Board Certification in Internal Medicine or Family Medicine
- Active, unrestricted Michigan medical license
- CPR certification (or willingness to obtain)
Preferred:
- Geriatrics and/or palliative care experience or certification
- Experience caring for medically complex or frail older adults
- A physician who values relationship-centered care
- Drawn to collaborative, team-based medicine
- Seeking a sustainable pace of practice
- Passionate about improving the lives of older adults
