Position Purpose

What You Will Do
  • Provide care across multiple patient homes within designated service areas or regions.
  • Incorporate and demonstrate standard elements of professional practice:
    Assess, Outcomes Identification/Solutions, Plan, Implement, Evaluate.
  • Act independently within license, scope of knowledge, and experience.
  • Retain accountability for delegation, decisions, and outcomes.
  • Collaborate with key stakeholders and contribute to quality improvement.
  • Exhibit agility and willingness to take on new responsibilities.
  • Embrace new ideas and cultural differences while managing competing priorities.
Minimum Qualifications
  • Current Registered Nurse license in the state of practice (no flags or restrictions).
  • Current BLS Certification from the American Heart Association.
  • Minimum of two years of acute care nursing experience.
  • Current Driver’s License, reliable transportation, and automobile insurance.
  • Home care experience preferred.
Preferred Clinical Skills & Experience
  • Foley and supra‑pubic catheter insertion, maintenance, troubleshooting.
  • Pleurex catheter drainage and site care.
  • Wound care, including Wound V.A.C. therapy.
  • PICC line dressing changes – sterile technique and site monitoring.
  • G‑tube management – feeding, flushing, site care.
  • Medical management – general patient care and chronic condition support.
  • IV insertion and administration.
  • Lab draws – venipuncture and specimen handling.
