RN Case Manager Inpatient

Overview

The RN Case Manager coordinates care for specific patient populations across the continuum and between care settings during an episode of illness. The RN Case Manager collaborates with physicians, the health care team, and patient/families to assess, plan, monitor, and evaluate ongoing care. The RN Case Manager is accountable for the ongoing monitoring and evaluation of patient outcomes.

• Coordinate delivery of resources for effective, efficient, and appropriate patient care during an entire episode of illness.
• Collaborate with physicians, health care team members, and patient/families to plan and evaluate care across the continuum and between care settings.
• Monitor patient outcomes and adjust care plans as needed to achieve optimal results.

• Education
  • Graduate of an accredited school of nursing.
  • Bachelor of Science degree in Nursing preferred or equivalent work experience
  • Masters Degree in Nursing (or actively pursuing) preferred.
• Licensure
  • Current licensure to practice nursing in Michigan
  • One year of experience at Trinity Health and two years of clinical experience in nursing
• Experience
  • Prior experience in Med Surg preferred
