** Position
Purpose:
*
* The Wound Ostomy Nurse oversees and coordinates care for specific specialty area for an assigned caseload. The Wound Ostomy Nurse facilitates appropriate care, assists patients and families in finding and negotiating that care, and facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and cost-effective care.
** Shift Available:
** Monday - Friday, No Holidays, No Weekends
** Status Available** :
Full Time 40 hours per week

* Required Qualifications:
*
* Education
+ Graduate of an approved (NLN or AACN) accredited school of nursing. Bachelor of Science in Nursing required.
Licensure /
Certifications:
+ Current RN licensure in the State of Michigan.
+ Specialty certification in specialty area preferred and/or required to obtain within 12 months of hire date.
Experience
+ Three (3) years of nursing experience required, previous case management experience preferred.
** What will you do:*
* + Complete a comprehensive health assessment of the patient's physical, psychological, social, environmental, financial and functional status. Includes assessment of past and present treatment course. Able to understand and develop goals, treatment plan and provider options.
+ Collaborate with other clinical teams and medical staff in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for identified populations.
+ Serves as clinical expert related to populations served providing education and internal consultation to other staff.
+ Utilizes critical thinking and problem-solving skills to analyze and synthesize all data and formulate an appropriate case management plan that addresses the patient's individual and family needs.
+ Monitors and modifies the plan of care through an interdisciplinary and collaborative process in conjunction with all parts of the health care team.
+ Educates patients and staff about the various procedures, potential complications and the continuum of care.
+ Participates in team conferences, rounds, or meetings in relation to care of the ostomy/wound care patient or program.
+ Ensures all ostomy/wound care intervention outcomes are monitored and addressed with variances.
