As a Home Health Licensed Practical Nurse (LPN), you will provide skilled nursing care to patients in accordance with the physician‑prescribed plan of care and under the supervision of a registered nurse. Care activities must comply with the state’s LP/VN/RNA Practice Act, licensure requirements, and agency policies.

• Graduate from an accredited or state‑approved school of practical nursing.
• Current licensure in the State of Michigan.
• CPR certification required.
• Minimum one year of experience in an acute‑care setting.
