* The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.

* Minimum qualifications:

* + Bachelor'sdegree in Nursing

+ Current Stateof Michigan Registered Nursing License .CPRorBCLSCertified.

+ Three years of experience in an acute care setting. One year of experience in medical, surgical or radiation oncology(foroncologyspecialty).

** What the Nurse Navigator will do:*
* + Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental,financialandfunctionalstatusincollaborationwithmultidisciplinaryteammembers.

Includesassessmentofpastandpresenttreatmentcourse.

+ Assistsinorganizingandcoordinatingtimelypatientappointments.

+ Understands and develops goals, treatmentplanand provider options in concordance withevidence basedstandards and clinical trials.

+ Worksdirectlywithphysicianleadtoreviewdiagnosticstudies,clinicalhistoryandcourseoftreatmenttoidentifymostappropriatenextcourse ofcare.

+ Collaborateswithmultidisciplinaryteammembersinperformanceimprovementactivitiesincludingassessment,planning,implementationandevaluationofclinicaloutcomesand processes for patients.

+ Usescritical thinking and problem solving skills to analyze and synthesize data and formulatean appropriate casemanagementplantoaddressthe patient'sindividual andfamilyneeds.
