- Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse
- Follow and carry out individualized Plans of Care established by the supervising RN
- Perform skilled nursing visits and complete accurate, timely clinical documentation
- Monitor patient status and promptly communicate changes to the supervising RN and healthcare team
- Educate patients and caregivers on medications, treatments, disease management, and self-care techniques
- Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes
- Participate in case conferences, staff meetings, and quality improvement activities
- Promote safe, compassionate, high-quality patient care while helping patients remain safely at home
- Competitive pay: $40 - $50 HOURLY

- Graduate of an accredited Licensed Practical Nursing (LPN) program
- Current, unrestricted Ohio Licensed Practical Nurse license
- Current CPR certification
- Minimum of one year of LPN experience required
- Home Health experience is preferred but not required
- Valid driver's license, reliable transportation, and auto insurance
