Position Type & Expected Hours of Work

This is a part‑time position that works on average 20-29 hours a week. Days and hours of work vary based on program needs. Most shifts will take place during regular business hours. Some weekday shifts could go until 7pm and some weekend shifts may be required (typically three hours per weekend shift).

Summary:

Community Living Supports (CLS) are used to increase or maintain personal self‑sufficiency, facilitating an individual’s achievement of his/her goals of community inclusion and participation, independence or productivity to increase Programs support individual’s goals identified in the IPOS. These supports are provided in their residence or in various community settings.

Essential Functions:

Essential functions of this position include:

  • Attend and participate in the Person‑Centered Planning (IPOS) process when invited.
  • Implement goals identified in the Person‑Centered Plans (IPOS) and other plans a participant may have by providing assistance, reminders, observation, guidance and/or training.
  • Record Service logs in Therap each time a service is provided to a participant. Complete other paperwork/documentation as requested by the SIL Homes Supervisor such as incident reports and Visa receipts and turn in requested paperwork in a timely manner as directed by the SIL Homes Supervisor.
  • Provide a culture of gentleness, dignity, respect, and a safe environment to all participants receiving Community Living Supports.
  • Ensure that all common living spaces indoors and outdoors are maintained to a standard that ensures the health and safety of the residents.
  • Assure all participants are aware of their rights.
  • Provide safe transportation from the program site to community activities, among community activities, and from the community activities back to the program site.
  • Attend all scheduled staff meetings and other meetings/In‑services requested by the SIL Homes Supervisor.
  • This position includes assisting participants in and out of vehicles, using the wheelchair lift, if required and securing belts and restraints and driving the vehicle.
  • A chauffeur’s license is required. Staff are reimbursed for chauffeur’s license if they do not have a prior one.
  • Responsible for reporting vehicle maintenance needs to the SIL Supervisor.
  • Provide personal care as needed.
  • Report all maintenance needs to the SIL Homes Supervisor.
