Peer Mentor

A Peer Mentor, under the supervision of IDD Program Supervisors, provides a welcoming atmosphere, skills training, and instruction to individuals receiving services who may be living with intellectual or developmental disabilities. An employee in this class builds relationships and meets with the individuals receiving services, motivating them toward greater community inclusion and achieving personal goals. This employee completes necessary documentation which includes communicating effectively in both oral and written form and maintaining accurate records, including utilizing basic computer software and performs other related duties as assigned.

Required

Minimum Entrance Qualifications

1. Be a high school graduate or have a certificate of completion of the General Educational Development (GED) test.
2. Have an intellectual/developmental disability. 3. Be willing to share their personal experiences to guide and support beneficiaries.
4. Possess a valid driver's license and the ability to obtain a Michigan Driver License within 30 days of employment.
5. Must have reliable transportation that may have to be used to carry out job duties of this classification. Post Hire Training Requirements
1. Must attend Michigan Developmental Disabilities Council's Peer Mentor 101 training.
2. Must obtain certification as a Peer Mentor through the Michigan Developmental Disabilities Council within 18 months of employment. This certification includes a 90-hour internship and certification through the DD Council. Preferred but not Required
1. Bilingual candidates highly desired.
2. Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
3. Individuals in Recovery and individuals with experience in Armed Services valued.

Physical Conditions / Work Location

PHYSICAL ACTIVITIES An employee in this class performs generally sedentary work activity requiring the occasional lifting of objects weighing thirty-five (35) pounds or less. ENVIRONMENTAL CONDITIONS A Peer Mentor works within the Client Services offices and program location of Health West, with considerable travel required throughout the County by use of personal resources.

Single Plan: $2,500 Deductible, $1,250 Employer HSA Contribution, Out of Pocket Max $4,000. 2 People/Family: $5,000 Deductible, $2,500 HSA Employer Contribution, Out of Pocket Max $8,000, Co-insurance – 10%
Employer Contributions are prorated based on start date of benefits

No Employee Premium Contributions Per Pay Prescription Drugs: $10/$40/$80 (After deductible)
Health Savings Account (HSA) Plan 2

Single Plan: $1,650 Deductible, $800 Employer HSA Contribution, Out of Pocket Max $2,250. 2 People/Family: $3,300 Deductible, $1,600 Employer HSA Contribution, Out of Pocket Max $4,500

Employer Contributions are prorated based on start date of benefits

Monthly Employee Premium:
Single-$161.58, 2 person- $387.80, Family- $484.76

Prescription Drugs: $10/$40/$80 (After deductible)
High Deductible Plan (HD)
* This plan is only optional if the employee is not eligible for the HSA,Single Plan: $750 Deductible, 2 People/Family: $1,500 Deductible, No Employee Premium Contributions Per Pay Prescription Drugs: $10/$40/$80

Available first of the month following one full month of continuous employment

Lincoln Financial 457b
- ** In lieu of employer HSA contribution you may be eligible for an employer contribution of 3% into the Lincoln Financial 457b plan

Dental Insurance - 15% employee premium contribution
