A well-established, mission-driven hospice organization serving the Muskogee area is seeking a compassionate, dedicated Licensed Social Worker to join our interdisciplinary team.

What You'll DoAs a Hospice Social Worker, you'll play a vital role in helping patients and families navigate the emotional, social, and practical challenges that accompany serious illness by:
- Providing compassionate counseling and emotional support to patients and their loved ones.
- Completing psychosocial assessments and developing individualized care plans.
- Connecting patients and families with community resources, financial assistance, advance care planning, and supportive services.
- Advocating for patients while serving as a trusted guide throughout the hospice journey.
- Collaborating closely with nurses, physicians, chaplains, aides, and other interdisciplinary team members to deliver exceptional, patient-centered care.
- Supporting families through anticipatory grief, bereavement, and difficult life transitions.
Required Qualifications
- Master of Social Work (MSW) from a CSWE-accredited program.
- Current Oklahoma Social Work license (LMSW or LCSW preferred).
- Minimum of one (1) year of healthcare experience (hospice, home health, palliative care, or related setting preferred).
- Excellent communication, organization, and interpersonal skills.
- Current CPR certification (or willingness to obtain).
- Valid driver's license, reliable transportation, and current automobile insurance.
