Our mission-driven and member-focused company is seeking an experienced and self-motivated Community Guide who has worked with individuals living with health challenges and social issues, and understands the unique needs of this community.

Role overview: This energetic and adaptable Community Guide is an integral part of Reema, a start-up transforming how health care and social care work together. The Guide builds genuine, positive relationships with members to improve health and wellbeing, using an integrated technology platform to call/text members, schedule community visits, and document interactions. The Guide manages the full lifecycle of their caseload—from initial outreach to ongoing engagement and program graduation.

Key strengths include excellent verbal and written communication, relationship building, problem solving, and comfort navigating a growing startup.

Work expectations: Significant time in the field, directly interacting with members who may have experienced trauma and face barriers related to mental, physical, or chemical health.

Responsibilities
  • Engage and work with clients from diverse ethnic backgrounds, focusing on those with complex social needs and chronic health conditions.
  • Reach out to members to enroll them in Reema Health care model.
  • Assess member needs and supports, including social determinants of health, to create member-centered care plans.
  • Establish trusting relationships and provide timely, quality support to achieve wellbeing goals.
  • Coordinate referrals and schedule/attend appointments with behavioral health, primary care, and other providers.
  • Manage a caseload of 60–100 members through cold outreach and provide coverage for other Guides when needed.
  • Set monthly goals and independently organize workload with effective time management.
  • Listen actively, maintain professional boundaries and confidentiality, and seek multidisciplinary consultation for complex cases.
  • Visit members in person as appropriate (home, community, hospital, or other treatment settings).
  • Communicate via text, phone, email, and in person, adapting to members’ preferences.
  • Identify and connect members to community and health resources; transport members as needed.
  • Prepare members for service endings, promoting self-sufficiency and independence.
Documentation
  • Document all work with members in the Reema App in a timely and thorough manner, including keeping care plans updated.
  • Maintain confidentiality and HIPAA compliance; support accurate member records in Reema’s technology.

  • Operate independently and collaboratively in a fast-paced startup environment and adapt to change.
  • Serve diverse community members with dignity and respect; build relationships with community resources and customers.
  • Collaborate with Reemates across the team to share perspectives and learn from others.
Education, Experience and Skills
  • Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related fields (experience may substitute for degree).
  • Three+ years of direct experience with the target population and knowledge of the community.
  • Commitment to working with individuals with complex health needs and belief in recovery.
  • Experience with crisis planning, case management, boundaries, and personal safety in community work.
