Position: Physical Therapist - Home Health- Florence ($15,000 Sign On Available)

Overview

Physical Therapist - Home Health (Sign On Available) at McLeod Health. This role involves delivering physical therapy services in accordance with physician orders and collaborating with patients, families, agency personnel, and physicians.

Responsibilities
  • Responsible for providing physical therapy services in accordance with the physician’s written orders. Interacts daily with patients, family members and agency personnel and on a regular basis with physicians.
  • Directly accountable to the McLeod Home Health Administrator. Functions within agency and unit policies and procedures. Manages own time productively and independently and complies with attendance and reporting requirements. Works closely with other staff, students, and external agencies and groups. Contributes to department performance through professional development strategies, including competency training, clinical preceptor, in-service coordination, mentoring in clinical expertise, and serving as clinical education coordinator for education of students and volunteers and assisting with performance improvement.
  • Demonstrates knowledge of state therapy laws, rules for Medicare/Medicaid (Title XVIII) and related regulatory requirements, ACHC, JCAHO, and other pertinent regulatory issues. Understands conflict resolution techniques and customer relations. Knowledge of agency services, referral systems, and community resources available to clients.
  • Demonstrates expertise in communication and interpersonal relationships. Possesses excellent writing skills, computer proficiency, and strong organizational abilities. Demonstrates adaptability, initiative, and sound judgment. Exhibits strong critical thinking and problem-solving skills.
  • Function in an office setting with ability to stand, walk, sit, and operate office equipment. Demonstrates manual dexterity for clinical care and equipment operation. Maintains good hearing and eyesight. Ability to perform moderate physical activity including walking, standing, bending, lifting up to 50 pounds, sitting, and driving. Full range of motion and strenuous movements may be required for patient care.
  • Responsible for addressing safety issues for staff and patients across settings. Demonstrates maturity, self-motivation, creativity, initiative, and independent judgment. Maintains ethical, self-initiating professional behavior. Communicates effectively with diverse populations and demonstrates empathy, listening, and planning with staff, patients, physicians, and customers. Ensures accurate data collection, record completion, and follow-up.
  • Familiar with growth and development principles across the lifespan and able to assess data reflecting patient status to meet age-specific needs.

Qualifications
  • Zero to one year of related work experience.
  • Must possess a current valid South Carolina driver’s license and automobile insurance as required by law. Must be at least 21 years old to be covered under McLeod Health’s fleet vehicle insurance policy.
Licenses and Certifications
  • Active South Carolina license in Physical Therapy
  • Current Basic Life Support certification
