Key Responsibilities- Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
- Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
- Educate patients and families on health management, medications, and self‑care techniques.
- Collaborate with the healthcare team to ensure the highest quality care for each patient.
- Assist with assessment, planning, implementation, and evaluation of care according to patient age and diagnosis.
- Communicate information in accordance with agency policies and procedures.
- Contribute to program effectiveness and maintain customer satisfaction.

- Education: Graduate of an accredited or approved practical nursing school.
- Licensure / Certifications: Current NC practical nurse license and CPR certification.
- Experience: Minimum one year of practical nursing experience; home health experience preferred.
- Interpersonal Skills: Ability to develop positive interactions with patients, families, physicians, and staff.
- Essential Physical Requirements: Ability to transfer patients, lift objects up to 50lb, and perform activities that require significant physical effort.
- Essential Mental Abilities: Ability to assess condition, develop care plans, evaluate responses, prioritize duties, and teach patients.
- Essential Sensory Requirements: Visual and auditory ability to assess patients and use equipment.
- Exposure to Hazards: May encounter blood‑borne pathogens, needle punctures, and weather extremes.
- Hours of Work: Variable days Monday‑Friday, with weekends and holidays as needed.
- Population Served: Adolescents, adults, geriatrics, and pediatrics.
- Transportation: Valid North Carolina driver’s license and operational vehicle.
