License Practical Nurse

General Description

The purpose of this job is to provide professional nursing care within an assigned unit and coordinate care planning with other disciplines.

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

• Obtain information including histories, vitals and lab specimens. Perform physical assessment after patient is evaluated by RN or MD. Participate in administration of vaccines.
• Assist in procedures and participate in wound care.
• Evaluate outcomes of patient care, consult with other specialists as required and revise nursing care processes as necessary to obtain optimal patient care.
• Coordinate discharge planning.
• Monitor and record patients’ condition. Maintain confidentiality.
• Perform charge duties as requested by the director, after one year experience and appropriate training as required.
• Round on patients per policy to improve clinical outcome.
• Orient or mentor new staff members.
• Listen effectively to the patient to facilitate care requirements.
• Perform other miscellaneous and related duties as assigned by the director of nursing, charge nurse, and/or medical staff.

Graduate of a National League of Nursing accredited school.

Current license to practice professional nursing in the state of South Carolina, a copy of which shall be maintained on file in the facility at all times.

A minimum of one year of professional nursing experience; some clinical or hospital experience is preferred, but not required.

CPR certification must be obtained within 6 months of hire.

Physical requirements are primarily related to the essential functions of any job. In order to perform the essential functions of this position you must be able to stand up to twelve hours, walk, sit, use hands, reach, stoop, kneel, talk, hear, and smell. Must be able to lift or exert energy up to 35 pounds. Close vision (clear vision at 20 inches or less), distance vision (clear vision at 20 feet or more), color vision (ability to identify and distinguish colors), peripheral vision (ability to observe area that can be seen moving up/down or left/right), and depth perception (three-dimensional vision, ability to judge distances).

Regularly exposed to the risk of blood borne diseases. Contact with patients under a wide variety of circumstances. Exposed to unpleasant elements including accidents, injuries and illnesses. Subject to varying unpredictable situations. Must be able to handle emergency or crisis situations. May perform emergency care. Occasionally subject to irregular hours. Occasional exposure to radiation.
