Certification Details
- 13 weeks contract length
- 36 hours per week
- Night shift
- 7 days on / 7 days off rotation
- Holiday rotation - work holidays that fall on their weekly rotation
- Medical Laboratory Technician (MLT) position requiring ASCP certification
Job Details
- MLT (ASCP) certification required
- American Heart Association BLS certification required for all direct patient care personnel
- License/Certificate verification must be dated within 30 days of start and attached
- Fit Test required (Iowa - 1860 mask only)
- Trinity Code of Conduct Form must be signed and on file
- Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement must be signed and on file
- Trinity Integrity and Audit Services Form must be signed within days prior to start
- Trinity Motor Vehicle Report (MVR) must be dated within 30 days of start and attached
- Trinity Color Vision test required
- Proof of MMR vaccination or IgG titer required
- Proof of Hepatitis B vaccination series required (titers not accepted)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required with chest xray if positive TB
- Proof of Varicella vaccination or IgG titer required
- Physical/Health Statement from exam within last year required
- Resume, Skills Checklist within a year, and 2 Professional References required
- Trinity Conflict Of Interest Disclosure Form must be signed and on file
- Trinity Criminal Background Check dated within 30 days of start required
- Trinity Education Verification required
- Trinity EPLS report dated within past 30 days required
- Trinity GSA/OIG report dated within past 30 days required
- Trinity Joint Commission Testing (CORE Exam within a year) required
- Trinity NSO report dated within 30 days of start required
- Trinity System Access Form must be completed and uploaded one week before start
- Trinity Skills Checklist required for clinical contractors
- COVID-19 Vaccine proof or declination required (declination not accepted for CA or IL)
- Influenza Vaccine proof or declination required (declination not accepted for CA or IL)
- 10-panel drug screen completed within 30 days of start required
- Standard i9 completed by the agency required
Job Requirements

Schedule Information
- American Heart Association certification required for all direct patient care personnel
- Must attach scanned copy of certifications with expiration dates to candidate's record in TrinityVMS
- Must upload state verification for licenses if available
- All engaged contractors must have required signed forms on file in TrinityVMS
- Must provide documentation and scanned reports as specified for various requirements
