Position: In-Home Nurse Practitioner or Physician Assistant (Full Time) - Boise, ID

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations and help bridge gaps in care.

Role Overview

The visit, which lasts about 45 minutes, allows you to connect one‑on‑one with the patient, answer health questions, review medication and medical history, perform a physical evaluation, and order additional diagnostic tests such as diabetic eye exams or spirometry if the health plan authorizes. You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine or alter a patient’s current treatment regimen.

  • Visit millions of people nationwide to connect them to the right care for their unique needs.
  • Work with Signify Health as a clinician colleague, fulfilling the specified role requirements.
  • Be flexible to travel locally and within licensed states as needed.
  • Conduct virtual visits as required.
  • Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, bone density screening, and others.
  • Perform visits in patients’ homes, ensuring safe and comfortable delivery of care in environments that may contain pets, smoke, or allergens.
Qualifications
  • Active, unrestricted license(s) in the coverage area(s).
  • Board certification (required).
  • Multi‑state licenses (preferred).
  • For NPs and PAs: relevant clinical experience and ability to work independently in a mobile, in‑home setting.

Pay Range

The typical pay range for this role is: $78,331.00 - $. The actual base salary offer will depend on experience, education, geography and other relevant factors.
