Home Health Case Manager – Meridian,
We are seeking an experienced Home Health Registered Nurse to serve as a Case Manager for a 13‑week assignment beginning 27 July 2026 in Meridian, travel nursing role offers flexible 5x8 hour days and a weekly pay rate of $2159.
Responsibilities- Collaborate with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload.
- Valid Registered Nurse license in the state of Idaho.
- Experience in home health or case management preferred.
