The Nampa family Justice center exists to promote safety self-sufficiency, hope and healing to those affected by abuse. The overall goal of the Safe Teen Assessment Center is to provide case management to youth and their family by implementing a systems approach to reducing duplication, filling gaps and needs, and increasing accountability by utilizing the Assessment Center Framework; divert as many youth as possible, consistent with public safety, from entering or going deeper into the child welfare and juvenile justice system;
improve youth outcomes through early intervention, comprehensive assessment, case management, and referral to supportive services and positive strategies tailored to address each youth and family’s individual needs; and serve as a resource and facilitate better relationships between law enforcement, child welfare, and agencies serving youth and families, and the community. The Case manager will proactively reach out to youth and families to encourage them to participate in the case management process and develop individualized plans that match them to quality and effective services and supports in the region.
Functions
- Maintain a monthly caseload of 25+ at-risk youth aged 10-17 years old (and not more than 50 youth).
- Follow-up with youth and families within 1 week of the development of the individualized case management plan, sooner, if necessary, based on the youth and family’s needs.
- Facilitate remote/virtual or face-to-face contacts, home visits, and accompaniment of youth and families to provider's when necessary to ensure access.
- Regularly meet with youth and their family to provide continuous support, check-in, and feedback.
- Utilize Find Help Idaho .org to coordinate referrals and conduct case management services.
- Link and refer youth and families to a variety of services based on their individual needs and what is outlined in the individualized case management plan.
- Contact and monitor service and support referrals to ensure the youth and their family’s needs are being met.
- Maintain communication with Assessment Provider and report on status of activities outlined in the individualized case management plan and any concerns or barriers.
- Connect and build working relationships with professionals and agencies to provide adequate referrals to services and supports.
- Participate in SWDH sponsored trainings and conference calls to ensure fidelity of the Safe Teen Assessment Center model.
- Participate in related meetings, trainings, webinars, conferences, workshops, and training sessions and reviews publications and audio-visual materials to become and remain current on principles, practices, and new developments in assigned work areas.
- Attend ongoing trainings and opportunities to understand trauma and the impact it has on victims and to learn more in the field of domestic violence (DV), sexual assault (SA), child abuse (CA), elder abuse (EA), stalking, and sex trafficking (ST).
- Know the safety protocol for emergency situations.
- Input client data in CAC Care Net system.
- Monthly submit data collection and reporting requirements as assigned.
- Maintain ongoing interaction with supervisor, to include articulation of any question or concerns.
- Ensure that all services are maintained in accordance with agency’s mission and goals.
- Adhere to all NFJC policies, procedures, and safety protocols.
- Perform all other duties as assigned by the Executive Director.
- Keep the immediate supervisor and designated others accurately informed about work progress, including current and potential issues, and provide suggestions for improvements.
