The Wraparound Facilitator provides culturally responsive services and leads a collaborative planning process for youth and families who are referred through Child Welfare, Juvenile Probation, or County Behavioral Health departments. Services will be provided almost exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who could be required to be placed in out-of-home care or are stepping down from a higher level of care.

Your primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.

The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of out-of-home placement of their child. A family-based team addresses the needs of the entire family, not just the child of focus.

This family-based team consists of family members, Seneca's Therapists, Support Counselors, Family Partners, Facilitators and other community members who have an important role in the family.

Responsibilities
  • Act as the clinical leader for your caseload, providing culturally responsive services to youth and caregivers weekly to advance treatment goals
  • Provide psycho-education, coach caregivers, and coordinate care among providers
  • Identify Family Finding and engagement needs and establish Youth and Family teams consisting of natural supports for treatment planning and lifelong support
  • Conduct monthly Child and Family team meetings (CFTMs) in compliance with the Wraparound Model
  • Cultivate and sustain engagement and alignment with community members, county partners, and other Mental Health providers
  • Maintain accurate and timely documentation, including mental health notes, treatment plans, safety plans, outcome measures, and incident reports
  • Participate in the 24-hour on‑call support line system
  • Attend weekly individual or group supervision sessions and other mandatory internal staff meetings or training sessions
Qualifications
  • Bachelor's degree + 4 years of experience in Mental Health field
  • MHRS Certification required
  • Availability to work 2 on‑call shifts per month
  • Flexible schedule with ability to work evenings and weekends
  • Willing to respond to crisis situations and be part of a rotating emergency pager system, scheduled ahead of time
  • Must be at least 21 years of age
  • Own vehicle, Valid California driver's license and insurability with Seneca's automobile policy
  • Driving is required within the community to meet with clients, attend meetings or perform job duties at multiple locations within the specific area
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
  • Experience working with youth in out-of-home care preferred
Schedule
  • Full-time
  • Monday to Friday 9AM - 5:30PM
  • Availability to work 2 on‑call shifts per month
  • Starting at $37.01 - $39.51 per hour
    • Actual salary is dependent on credible experience above the minimum qualifications for the role
