Travel Physical Therapist - Home Health

American Traveler is seeking a Physical Therapist with 1+ year recent home health experience for a North Napa Valley assignment with required OASIS and HCHB proficiency.

Job Details

• Home health setting serving a primarily geriatric patient population,

• Work territory may extend up to a 60-mile radius from the office hub,

• Day shift schedule: 5x8 hour shifts, 8:00 to 17:30,

• Expected to complete 6 visits daily and a minimum of 30 points weekly for productivity,

• Average caseload is 5-7 patients per day,

• Required to be on-call every 3rd to 4th weekend,

Job Requirements

• Active PT license required if applicable,

• Minimum of 1 year recent home health PT experience,

• OASIS documentation experience required,

• Experience with HCHB EMR required,

• Current BLS certification required,

• Must be flexible with covering broad territory,

• Timely documentation and communication skills required,
