Referral Coordinator RN - IP Rehab Unit at Providence Queen of the Valley Medical Center in Napa, CA. This position is Part-Time and will work 8-hour Day shifts.

Under the general supervision/direction of the Acute Rehabilitation Nurse Manager, and in collaboration with the treatment team and external entities, the Rehabilitation Admissions Nurse shall monitor, coordinate and facilitate the interdisciplinary plan of care, effective clinical resource management, discharge planning, and continuation of post-acute care needs of patients assigned.

ResponsibilitiesCoordinate and facilitate the interdisciplinary plan of care, manage clinical resources effectively, and plan for discharge and continuation of post-acute care needs for patients assigned.
QualificationsRequired Qualifications
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 2 years - Rehabilitation Nursing.
Preferred Qualifications
- Certified Rehab Registered Nurse (CRRN) upon hire.
- Bachelor's Degree - Nursing.

Posted are the minimum and maximum wage rates for this position. The successful candidate’s placement on the wage range will be determined based on relevant experience and other factors. Base pay range may include additional compensation such as shift differentials, standby/on-call, overtime, premiums, and bonus opportunities.
