Salary Range
$62,244 to $68,652 with opportunities for 5% increases annually
DescriptionUnder the general direction of a Case Management Supervisor, the Service Coordinator manages the delivery of services to regional center clients. Responsibilities include implementing clients’ service plans, assisting clients in use of public and private resources, monitoring vendor services, evaluating the effectiveness of Individual Program Plans, ensuring appropriate community resources are made available, conducting annual reassessments, and occasionally performing the Lanterman eligibility determination for children under “Provisional Eligibility”.
The Coordinator may act in a supervisory or lead capacity, e.g., Officer of the Day or Acting Supervisor.
- Implement clients’ service plans.
- Assist clients in using public and private resources before using regional center funds.
- Monitor vendor services and verify delivery in accordance with service authorizations.
- Evaluate the effectiveness of Individual Program Plans.
- Ensure appropriate community resources are available to the client.
- Conduct annual reassessments of clients’ developmental status in conjunction with the interdisciplinary team.
- Assist Intake with determining Lanterman eligibility for young children ages 3‑6.
- Occasionally act in a supervisory or lead capacity.
- Master’s degree in a field related to developmental disabilities.
- OR Bachelor’s degree in a related field (e.g., social work, psychology, counseling, special education, nursing) plus three years employment in a related field. Minimum education requirement may not be waived.
- Fluency in a language other than English is desirable and may be required.
- Bilingual (English/Spanish preferred).
- Ability to provide personal automobile transportation during working hours and after‑hours emergencies or access to reliable transportation meeting minimum legal requirements.
All employment requires face‑to‑face, in‑person interactions. Candidates must live within commute distance to the preferred NBRC office and assigned clients. A hybrid work arrangement may be available after at least 3 months of in‑office work.
