Summary Under close supervision, the Transitions Case Manager provides support and structure to youth in developing program plans by managing a caseload and administering the policies and procedures of the program as directed by the contracting agency and the Program Manager. Learns, uses, and models the principles of the "What Works" system through evidence‑based practices in all interactions with participants and other staff members.
Responsible for working with youth in developing transition plans to help them be successful upon their release from custody.
- The Transitions Case Manager prepares written personalized programs for each youth to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals.
- Reviews plan with youths and documents progress or revisions so that they are gaining the life skills necessary to make a successful re‑entry into the community.
- The Transitions Case Manager works with youth to create a transition plan that will aid in their success upon release.
- Responsible for connecting with community agencies who can partner and provide services to youth both in and out of custody.
- The Transitions Case Manager may work with youth’s families to assist with stabilization and successful reentry.
- Assists youths to establish personal budget, locate suitable housing, enroll in educational and vocational programs and participate in family and community activities, as appropriate, to ensure that they are working toward financial independence.
- The Transitions Case Manager may orient youths upon arrival at facility to provide smooth assimilation into program including having them complete paperwork.
- Make entries into confidential youth files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensure files are secure to protect youth confidentiality.
- The Transitions Case Manager verifies youths' pass plan through pass sponsorship according to contract so that the youth is accountable for his/her whereabouts in the community.
- Prepares discharge reports within established time parameters upon youth release to summarize program performance and to satisfy contract requirements.
- Participates in staffing meetings by sharing observations and recommendations regarding the facility, youths, and staff relations to identify and solve problems and function as a unified team.
