In-Home Health - Physician; Part Time - Fort Collins

Job Summary

Signify Health, part of CVS Health, is seeking a Part‑time Physician to provide In‑Home Health Evaluations. Physicians will visit patients in the comfort of their own homes to conduct comprehensive health assessments and connect them with needed care.

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Conduct in‑home visits (approximately 45 minutes each) that include medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the patient’s health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each evaluation; prescription, lab ordering, or treatment changes are not required.
• Travel locally and within licensed states as needed; conduct virtual visits when appropriate.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams.

• MDs or DOs only.
• Active, unrestricted license(s) in the coverage area(s).
• Multi‑state licenses are preferred.
• Ability to cover the primary location of Fort Collins and surrounding area.
• Comfortable and capable of working in diverse home environments.

Anticipated weekly hours: 29.5. Pay is based on a base hourly rate of $105.00 (same for all locations). Additional reimbursement is available for completed Diagnostic and Preventive Services.

Part‑time physicians qualify for selective benefits, including malpractice insurance coverage during Signify Health‑related activities and supplies. Specific benefits such as dental, vision, wellness resources, and employee discounts will be disclosed during the application process.
