Lifecare Home Health - - - Responsibilities:
Assist clients in their place of residence with personal healthcare and health maintenance activities, including transferring, mobility, feeding, and exercises.;
Monitor and assess patient condition, record observations, and collaborate with the care team to develop and implement a plan of care.;
Provide housekeeping or laundry services, shopping, and meal preparation to support the patient.;
Provide personal care such as bathing, dressing, and grooming; educate family on treatment and aftercare needs.;
Record patient information via EMR and notify branch leadership of changes; participate in on-call responsibilities when assigned.
