Location: Siena Lakes by Erickson Senior Living, 2521 Orange Blossom Dr, Naples, FL 34109

Compensation: $45,000 - $51,000 per year (includes base + bonus + incentive)
Work Schedule: Part‑time;
Saturday and Sunday 7 AM – 7 PM (alternate schedules available: Monday‑Friday 8:00 AM – 4:30 PM, Wednesday‑Friday 8:00 AM – 4:30 PM)
- Licensed Social Worker: Provide social work services to residents and family members in skilled nursing, long‑term rehab, assisted living, and memory care; complete psychosocial assessments, MDS, and other required documentation in accordance with department policy, state, and federal regulations.
- Resident Services Coordinator: Conduct assessments of prospective residents; recommend appropriate level of residency; provide psychosocial support to residents and families and assist through transition process to higher levels of care.
- Resident Services Coordinator II: Engage with residents, respond to psychosocial or supportive service needs, direct referrals, and review incident reports.
