Location:

Ameri Care Home Health Care - Naples, FL 34105.

Salary Range: $81,000.00 - $ per year. Sign‑on Bonus: $10,000 for qualified candidates.

Territory:
Naples to Marco Island. Mileage reimbursed at the current federal rate of 72.5 cents per mile.

Job Summary

The licensed Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals. The PT will also maintain a positive working relationship with other health professionals and ancillary staff.

Responsibilities
  • Provide PT services prescribed by a physician, which can be safely provided at home:
    • Initial and ongoing assessments to determine level of functioning, including OASIS assessments (G333, G336).
    • Developing and revising the plan of care with physician and care‑team members (G162, G186).
    • Providing therapeutic treatments and evaluating equipment needs as applicable.
    • Advising and consulting with family and agency personnel regarding medical equipment, HEP, safety issues (G143, G188).
    • Preparing clinical and progress notes (G187).
    • Evaluating outcomes of care.
    • Supervising Therapy Assistants and Home Health Aides as appropriate (G190).
    • Planning for discharge.
  • Follow all policies and procedures, including safeguarding patient privacy and the Compliance Program.
  • Participate in in‑services and QA programs as requested (G189).
  • Complete all other duties as assigned.

Qualifications
  • Graduate of an accredited PT program.
  • Minimum 1 year experience in Physical Therapy.
Certificates, Licenses, Registrations
  • Valid Florida Driver’s License.
  • Licensed Physical Therapist in Florida.
  • Valid CPR certification.
Computer, Language, Mathematical, Reasoning Skills

Knowledge of basic typing, data entry, word processing software, database software, and general e‑mail usage.

Ability to read, analyze, and interpret scientific and technical journals, financial reports, and legal documents. Ability to write speeches, articles, and present information to various audiences.

Ability to apply mathematical concepts such as probability, statistics, fractions, percentages, ratios, and proportions.

Ability to define problems, collect data, establish facts, and draw valid conclusions; interpret technical instructions.

Physical Demands

Regularly sit, use hands to finger, handle, or feel, talk or hear; frequently stand, walk, reach; occasionally stoop, kneel, crouch, crawl; lift up to 10lbs and occasionally up to 25lbs. Vision abilities include eye focus and perception.

Work Environment

Occasional exposure to weather conditions; moderate noise level.
