Job Summary

The Clinical Specialist Pharmacy Maternal, Neonatal & Pediatric Medicine is responsible for the provision of safe, effective, evidence-based medication management. Pharmacotherapy services provided include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Provides direct patient-centered care and integrated pharmacy services in practice setting with physicians, nurses, and other hospital personnel.

Are aligned with the target interdisciplinary programs and services to deliver medication therapy management to ensure maternal, neonatal, and pediatric pharmaceutical care programs are appropriately integrated throughout the health system. Serves as departmental resource and liaison to other departments, hospital personnel, or external groups. Provides medication and practice-related education/training to staff members and actively serves as preceptor for Doctor of Pharmacy (Pharm.D.) students and pharmacy residents.

Participates in clinical pharmacy committees and multidisciplinary committees. This position reports to the System Pharmacy Clinical Manager.

Essential Duties and Responsibilities
  • Other duties may be assigned.
  • Provides direct medication therapy services and pharmaceutical patient care to assure and promote drug utilization activities that are aligned with patient care needs, evidence-based practices, and regulatory standards.
  • Maintains proficiency in pharmacy services and clinical pharmacy programs.
  • Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care.
  • Identifies high-risk patients and implements interventions to improve quality and safety.
  • Makes appropriate evidence-based, patient-centered medication recommendations.
  • Participates in the management of medical emergencies.
  • Provides accurate safe, timely medication review, reconciliation, and counseling as appropriate.
  • Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs.
  • Maintains proficiency in hospital computer systems and medication ordering systems.
  • Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmacotherapy care between shifts and among staff.
  • Actively participates in stewardship activities and restricted medication programs.
  • Facilitates medication procurement, ordering, and dispensing procedures.
  • Oversees pharmacy operations and technician practice/activities as defined by state laws.
  • Participates in clinical pharmacy committees and multidisciplinary committees.
  • Identifies, designs, implements, and tracks improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs, routinely benchmarks against local and national best practices.
