Job Description:
We are seeking a highly skilled and empathetic Hospice Care Specialist to join our team. The ideal candidate will possess extensive experience in providing end-of-life care, demonstrating both clinical expertise and a deep understanding of the emotional and psychological needs of patients and their families.
Key Responsibilities:
Deliver comprehensive hospice care to patients/clients in their homes.
Provide emotional support to patients/client and their families.
Administer medications and treatments as prescribed, ensuring patient comfort and symptom management.
Conduct regular assessments and report to DON for adjustments to the care plans as needed to meet changing patient/client needs.
Educate patients/clients and families about the hospice care process, addressing any questions or concerns.
Maintain accurate and detailed patient records in compliance with regulatory standards.
Qualifications:
Licensed Practical Nurse (LPN) license.
Minimum of 3 years of experience in hospice or palliative care.
Strong clinical skills and the ability to manage complex medical conditions.
Excellent communication and interpersonal skills.
Empathetic and compassionate nature with a strong commitment to patient-centered care.
Ability to work independently and as part of a collaborative team.
Proficiency in electronic medical records (EMR) systems.
