We are currently seeking a full-time Physical Therapist to join our team. The position offers a competitive salary of up to $105K, dependent on experience. This is a great opportunity for a skilled and compassionate Physical Therapist to make a positive impact in the lives of our patients.

As a Physical Therapist with Trilogy Home Healthcare, you will provide skilled therapy services to geriatric patients who live r primary responsibility will be to assess the patient's condition and develop a personalized treatment plan to help them regain their mobility and independence. You will work closely with the patient's physician and other healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities:

  • Conduct comprehensive assessments to evaluate older adults needs and establish personalized care plans.
  • Implement physical therapy interventions based on the latest evidence-based practices.
  • Monitor patient progress and adjust care plans as necessary.
  • Educate patients and families on exercises, mobility aids, and strategies to improve daily living.
  • Coordinate with other healthcare professionals to provide holistic care.
  • Maintain accurate and timely documentation of treatments and outcomes.

What you need:

  • Valid Florida Physical Therapy license and degree from an accredited PT Program
  • Exceptional interpersonal and communication skills.
  • Reliable transportation to visit patients in the specified areas.
  • Previous experience in geriatric physical therapy preferred.
  • A strong commitment to patient-centered care.
  • Ability to work independently with a flexible schedule.
  • CPR certification
  • A positive attitude, integrity, compassion and care
