Position Summary Moorings Park is seeking a dedicated and talented Physical Therapist to join our team. The Physical Therapist is responsible for insuring and coordinating quality and safe delivery of home health physical therapy services following applicable licensure/certification requirements both State and Federal. The Physical Therapist has responsibility for the planning and therapeutic interventions of all physical therapy cases. Physical therapy will be provided by a licensed Physical Therapist or a licensed Physical Therapy Assistant, under the supervision of a licensed Physical Therapist, in compliance with the Plan of Care.
Key Contributions
- Performs physical therapy assessment including evaluating the patient's needs, including medical, psychosocial, and environmental needs. When no skilled nursing is necessary, performs initial client assessment, obtains necessary authorizations, consents, and intake data.
- Develops and implements, with physician, patient and the Clinical Manager, an individualized Plan of Care based on the patient's physical, medical, psychosocial, and environmental needs, and makes appropriate revision as the patient's condition changes.
