Role: Patient-centered care coordinator responsible for staffing, scheduling, and communication within a healthcare team.
Key Responsibilities:- Promote company mission, vision, and values
- Ensure timely staffing and scheduling of field staff based on client preferences
- Follow up with staff, patients, families, and physicians regarding care reports and communications
- Maintain accurate client rosters
- Provide effective communication to clients, families, and healthcare professionals
- Participate in professional development and maintain competency
- Schedule caregivers to meet client needs
- Deliver concierge-level customer service to employees and clients
- Participate in on-call rotation
- At least 1 year of experience in a general office environment, healthcare experience preferred
- Effective verbal and written communication skills
- Proficiency with computer and office technology
- Reliable transportation
- Experience with home care scheduling is preferred
