The Arlington of Naples is seeking a compassionate, hospitality-focused Social Worker to join our Health Center team.

Position Summary

The Social Worker plays a vital role in supporting the emotional, psychosocial, and transitional needs of residents. This position serves as the licensed Social Worker for the Health Center, ensuring residents receive individualized support and advocacy in accordance with community policies, professional standards, and applicable federal and state regulations.

The ideal candidate is a strong communicator, compassionate listener, and collaborative team member who is committed to enhancing resident well-being and quality of life.

What You'll Do
  • Conduct comprehensive social history assessments to identify residents' strengths, needs, preferences, and opportunities for support.
  • Assist with Health Center admissions, assessments, and resident transitions into the community.
  • Help residents and families adjust to their new environment by providing orientation, resources, and ongoing support.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that address psychosocial needs.
  • Participate in care plan meetings and encourage resident and family involvement in the care planning process.
  • Ensure advance directives are completed, maintained, and appropriately documented.
  • Complete all required assessments, documentation, and reports accurately and within established timelines.
  • Maintain strict confidentiality of resident information in compliance with HIPAA and community standards.
  • Build trusted relationships with residents and families, addressing concerns and facilitating effective communication between all parties.
  • Coordinate resident discharges and transfers to support continuity of care and successful outcomes.
  • Connect residents and families with community resources, referral agencies, and support services.
  • Facilitate support groups, advocate for resident rights, and contribute to policies that positively impact resident life and well-being.
What You'll Bring
  • Licensed Social Worker (LSW) required.
  • Experience working with older adults, long-term care, skilled nursing, rehabilitation, or healthcare settings preferred.
  • Strong assessment, counseling, advocacy, and case management skills.
  • Ability to build meaningful relationships with residents, families, and interdisciplinary team members.
  • Excellent communication, organization, and documentation skills.
  • Commitment to delivering compassionate, resident-centered care.
