FACT Case Manager

Schedule
:
Monday through Friday, 8:30 a.m. to 4:30 p.m., with occasional evening shifts from 11:30 a.m. to 7:30 p.m. This position also requires participation in a rotating weekend schedule (11:30 a.m. to 7:30 p.m.), as well as rotating evening, holiday, and on-call coverage to support 24-hour service operations.

Job Summary: The FACT Case Manager is a member of a multidisciplinary Florida Assertive Community Treatment team that provides intensive, recovery-oriented behavioral health services to adults with serious mental illness and/or co-occurring substance use disorders. The position delivers most services in community settings and helps participants improve stability, reduce hospitalization and incarceration, strengthen independent living skills, and achieve individualized recovery goals.

Services are person-centered, strengths-based, flexible, and coordinated across behavioral health, medical, housing, benefits, vocational, legal, and natural-support systems.

Duties may include but are not limited to:

• Serve as the primary case manager for an assigned caseload while sharing responsibility for all participants through the team-based FACT model.
• Complete comprehensive assessments and recovery plans and contribute to individualized treatment, service, crisis, safety, and transition plans with participants, families, natural supports, and the interdisciplinary team.
• Provide case management, rehabilitation, skill building, supportive counseling, crisis intervention, advocacy, and care coordination in homes and other community settings.
• Assist participants with housing stability, benefits, transportation, budgeting, nutrition, hygiene, appointments, communication, social skills, and other activities of daily living.
• Support medication adherence and symptom self-management within the employee's scope of practice and in coordination with nursing and medical staff.
• Identify changes in mental status, functioning, safety, substance use, or medical condition; communicate concerns promptly and follow crisis and emergency procedures.
• Coordinate behavioral health, primary care, substance use, pharmacy, hospital, legal, vocational, educational, and community resources to support continuity of care.
• Provide education to participants and natural supports regarding recovery, symptoms, medications, relapse prevention, coping strategies, wellness, and available resources.
• Participate in daily team meetings, recovery planning, case reviews, inpatient discharge planning, and other care coordination activities.
• Complete accurate, timely, confidential documentation in the electronic health record and meet agency, payer, Medicaid, contractual, and regulatory requirements.
• Maintain required productivity, service frequency, outreach, follow-up, and quality standards.
• Transport participants when authorized and safe, using an agency vehicle in accordance with agency policy.
• Maintain professional and ethical boundaries, protect participant rights and confidentiality, and comply with HIPAA, safety requirements, and agency policies.
• Perform other duties as assigned to support program operations and participant needs.

• Bachelor's degree from an accredited institution in social work, psychology, counseling, rehabilitation, human services, criminal justice, or a related field.
• Experience providing behavioral health, substance use, rehabilitation, community-based, or case management services; experience serving adults with serious mental illness is strongly preferred.
