Position Summary

Moorings Park is seeking a dedicated and talented Reimbursement Nurse to join our skilled nursing team. The Reimbursement Nurse coordinates the appropriate participating health professionals for the purpose of conducting initial and periodic comprehensive, accurate assessments of each resident to plan the care that allows the resident to reach his/her highest practicable level of physical, mental, and psychosocial functioning. Also, conducts and coordinates the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS).

Key Contributions

  • Organizes and conducts the care plan process and conference with nursing staff, ancillary departments, residents, and families.
  • Completes MDS/PDPM requirements according to State and Federal guidelines.
  • Updates individualized care plans quarterly and on an as needed basis.
  • Maintains the care plan calendar and distributes to all appropriate personnel.
  • Participates in the weekly Medicare Meeting to review all residents for continued compliance with Medicare A classification.
  • Transmits required MDS/PDPM documents to the appropriate authority on a timely basis and as needed.
  • Assures all appropriate signatures are present on all care plan documents.
  • Oversees the monitoring and documentation of the residents' status in numerous physical, behavioral, activities of daily living, and treatment areas.
