Position Summary Moorings Park is seeking a dedicated and talented Wound Care Nurse (RN)to join our skilled nursing team. The Wound Care Nurse (RN) oversees the facility infection control and wound care programs. Conducts wound care rounds with the facility wound care provider, collaborates with the interdisciplinary team and clinical staff to establish and maintain treatment programs. Gathers and maintains the facility infection control program.

Key Contributions

• Consults and coordinates with the interdisciplinary team (IDT) and healthcare professionals to plan, implement and evaluate residents wound care treatments and monitor the residents progress and clinical needs.
• Participates in quality committees including quality assurance and performance improvement (QAPI), infection control, safety, risk management, etc.
• Leads the identification and mitigation for resident risk factors for pressure ulcers/injuries.
• Assists the interdisciplinary team in identifying and managing resident pain related to wounds.
• Provides skin and wound care and treatments, as ordered.
