We're looking for a Full-Time RN to provide skilled home health services to patients in assisted living, independent living, and other residential care communities in the Naples area.

What You'll Do- Perform comprehensive nursing assessments and admissions
- Develop and update individualized plans of care with physicians and the care team
- Deliver skilled nursing visits per physician orders and agency policy
- Monitor patient status and communicate changes promptly
- Educate patients, families, and caregivers on disease management, meds, and safety
- Complete OASIS assessments and documentation accurately
- Coordinate care with therapists, social workers, physicians, and facility staff
- Maintain compliance with Medicare, CMS, and ACHC standards
- Graduate of an accredited nursing program
- Current CPR/BLS certification
- 1+ year of RN experience
- Home health and OASIS experience preferred
- Strong assessment, communication, and organizational skills
- Comfortable working independently while collaborating with a team
