Licensed Practical Nurses; LPN PRN

Northcrest Rehab and Nursing Center in Napoleon, Ohio is seeking PRN Licensed Practical Nurses (LPNs) to join our team!

As a Northcrest LPN, you will provide direct nursing care to residents while working under the supervision of an RN or Director of Nursing. You will play a key role in ensuring quality care and resident comfort.

• Provide direct nursing care in accordance with care plans
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes
• Support and assist CNAs/STNAs with resident care
• Maintain accurate and timely documentation
• Promote and protect resident rights
• Follow all facility policies and procedures
• Perform other duties as assigned

PRN team members must have flexible availability, including days, nights, weekends, and holidays
, based on facility needs.

• Current LPN license in the state of Ohio
• Strong communication and clinical skills
• Long‑term care experience preferred
