Licensed Practical Nurse; LPN

Location: Fulton and Henry Counties, covering the Defiance area.

Under the supervision of an RN, you will assess, plan, and implement patient care; educate patients and families on disease processes, procedures, and medications; and address various patient needs, including wound care, trach, G/J tubes, Foley catheters, and medication administration.

We provide autonomy to use your knowledge, skills, and judgment; broad case diversity with high‑acuity cases; solid support with case manager accessibility, iPad/electronic charting, portable printers, and an open‑door management policy; and excellent training through orientation, skill labs, and the Elsevier Home Health Care mobile platform.

The Elsevier platform offers quick access to drug information, patient education, animations and videos of procedures, updates on best practices, and online assigned tasks and training.

As a member of our home‑health LPN team, you will build one‑on‑one, long‑term relationships and witness positive outcomes as patients regain health and independence.

Requirements:

• Current LPN state licensure
• Three years of nursing experience
• CPR certification
• Ability to travel for in‑home patient visits
• Ability to work occasional nights and weekends
• Exceptional time‑management skills
• Excellent interpersonal and customer service skills
• Home health care experience preferred
