Northcrest Nursing & Rehabilitation Center is seeking a compassionate, organized, and resident-focused Social Services Director to join our leadership team. This position plays an important role in supporting our residents and families while helping ensure a positive experience from admission through discharge.
Key Responsibilities:- Lead and oversee the facility’s Social Services department
- Advocate for residents and support their psychosocial and emotional needs
- Build strong relationships with residents, families, and responsible parties
- Participate in care planning and interdisciplinary team meetings
- Coordinate discharge planning and safe transitions of care
- Connect residents and families with community resources and services
- Address resident and family concerns promptly and professionally
- Complete required assessments, documentation, and care plan updates
- Support resident rights and regulatory compliance
- Collaborate with Nursing, Admissions, Therapy, Activities, and facility leadership
- Degree in Social Work, Human Services, or a related field preferred
- Previous social services experience in skilled nursing, long-term care, healthcare, or a related setting preferred
- Knowledge of resident rights and discharge planning is a plus
- Strong communication, organization, and problem-solving skills
