About the Job
An Ambulatory Registered Nurse in our Diabetes & Endocrinology practice plays a vital role in delivering specialized care to patients managing chronic endocrine conditions. The position involves collaborating closely with endocrinologists, advanced practice providers, and care teams to provide evidence-based, patient-centered care, including in-office care, telehealth services, and chronic disease management support.
What You'll Do- Deliver comprehensive nursing care using the nursing process (assessment, planning, intervention, evaluation) with a focus on chronic disease management.
- Support patients with diabetes care, including blood glucose monitoring, medication adherence, and lifestyle management.
- Educate patients on:
- Insulin administration and injectable therapies
- Continuous glucose monitors (CGMs) and insulin pumps
- Nutrition, weight management, and lifestyle modifications
- Collaborate with providers to develop and implement individualized endocrine care plans.
- Administer medications, vaccines, and perform point-of-care testing (e.g., A1C, glucose checks).
- Provide telephone triage and telehealth support, addressing patient questions and symptom management.
- Document care accurately in the EMR and assist with pre-visit planning.
- Monitor and follow up on lab results, medication adjustments, and care plan adherence.
- Serve as a resource and mentor to clinical staff; assist with onboarding and education.
- Respond to urgent clinical concerns in alignment with established protocols.
- Associate of Science in Nursing (ASN) required;
Bachelor of Science in Nursing (BSN) preferred. - Current Registered Nurse license in NH (or NLC state/MA) and BLS certification required.
- 1+ year of professional nursing experience preferred; ambulatory care experience a plus.
- Strong knowledge of nursing principles, EMR systems, and patient-centered care models.
- Skilled communicator with excellent time management, adaptability, and teamwork abilities.

Work Shift:
40 hours per week (10-hour shifts, Wednesdays off).
