About the Job

The Licensed Practical Nurse (LPN) plays an important role in delivering high-quality patient care within our ambulatory practices. Working under the supervision of Registered Nurses, Providers, and the Practice Manager, the LPN supports the nursing process, assists with clinical operations, and helps ensure an exceptional patient experience through compassionate, patient-centered care.

What You’ll Do
  • Participate in the nursing process through in-person visits, telephone triage, and telehealth encounters.
  • Assist with patient assessments, interventions, and evaluation of care plans.
  • Collaborate with physicians, advanced practice providers, registered nurses, and other healthcare professionals to support coordinated patient care.
  • Educate patients on medications, self-management, nutrition, health maintenance, and wellness initiatives.
  • Document patient care accurately and thoroughly in the electronic medical record.
  • Assist providers during examinations, treatments, and diagnostic procedures within scope of practice.
  • Administer medications, vaccines, and treatments; perform point-of-care testing; and collect specimens as appropriate.
  • Support preventive care and chronic disease management through pre-visit planning and patient outreach.
  • Maintain compliance with HIPAA, infection prevention, safety standards, and organizational policies.
  • Participate in ongoing education, competency programs, and professional development opportunities.
Who You Are / Requirements
  • Graduate of an accredited school of nursing required; OR currently enrolled in an accredited RN program and hold a current LPN license.
  • Current Licensed Practical Nurse (LPN) licensure in New Hampshire, a Nurse Licensure Compact (NLC) state, Massachusetts, or as required by the practice location.
  • Current BLS/CPR certification required.
  • Strong understanding of nursing principles, clinical protocols, and patient care best practices.
  • Working knowledge of electronic medical records and Microsoft Office applications.
  • Excellent communication, teamwork, and patient service skills.
  • Ability to multitask, prioritize responsibilities, and thrive in a fast-paced clinical environment.
  • Previous LPN experience and ambulatory care experience preferred, but not required.
