We provide exceptional, comprehensive surgical and nonsurgical care for injuries and conditions. Our experienced team offers exceptional, comprehensive care, tailored to each individual and their unique set of needs. Our experienced providers and staff work with patients to help them understand treatment options so that they can make informed decisions about their care. We treat our patients with integrity, compassion, and respect.

Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.

Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities. Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care. Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals. Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.

Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).

Graduate of an accredited nursing program (ASN required, BSN preferred). Licensed RN in NH, MA, or compact state, with current BLS/CPR certification. Prior experience in ambulatory care, anticoagulation management, or chronic care preferred. Strong communication and clinical assessment skills. Comfortable working independently and collaboratively in a fast-paced setting. Knowledge of EMR systems (EPIC preferred) and patient-centered care practices.
