Position: Staff - Registered Nurse (RN) - Hospice - $70K-90K per year

Job Summary

Gentiva is seeking a Registered Nurse (RN) in hospice care for a nursing position in Nashville, North Carolina.

Role Overview

As a Hospice RN / RN Case Manager, you will be the primary point of contact for patients and families, guiding them through every step of their hospice journey.

Key responsibilities include delivering and documenting skilled nursing care based on individualized plans, performing assessments, collaborating with an interdisciplinary team, providing emotional, physical, and spiritual support, teaching caregivers, communicating updates, maintaining documentation, supporting quality initiatives, and helping families navigate medication, hospice philosophy, symptom management, and end‑of‑life processes.

Responsibilities
  • Serve as primary contact for patients and families, guiding them through hospice journey.
  • Deliver and document skilled nursing care based on each patient’s Plan of Care in their home or assisted living settings.
  • Perform initial and ongoing assessments; monitor patient condition and adjust care as needed.
  • Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to build and refine care plans.
  • Provide emotional, physical, and spiritual support to patients and their loved ones.
  • Teach caregivers safe and confident self‑care practices, providing training and reassurance.
  • Communicate important updates in patient condition to the care team, ensuring timely interventions.
  • Maintain accurate, up‑to‑date clinical documentation; follow infection control and compliance standards.
  • Support quality‑improvement and scheduling initiatives for consistent, compassionate, efficient care.
  • Help patients and families understand medication administration, hospice philosophy, symptom management, and end‑of‑life expectations.
Essential Functions
  • Conduct comprehensive patient assessments via in‑person/telephonic screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized care plans, including visit frequency.
  • Coordinate care across interdisciplinary teams and provide consultations.
  • Educate patients and caregivers about disease processes, self‑care, medications, nutrition, and dietary needs.
  • Communicate changes in condition or signs of decline to providers (NP, PCP, hospice physician).
  • Support continuity of care by recognizing and facilitating referrals and transitions in levels of care.
  • Document clinical records: screening findings, orders, progress notes, and care plans.
  • Maintain effective relationships with IDG; involve team when needed.
  • Participate in on‑call and patient support activities in a competent, responsive manner.

Requirements

Education: Registered Nurse

Experience: Minimum of one (1) year as an RN in acute care, oncology, long‑term care, hospice, or home health. Hospice or palliative care experience preferred.

Licenses, Certifications and Registration:

  • Current license to practice nursing in the state of employment
  • CPR Certification
  • Valid driver’s license and automobile insurance

Specialized Knowledge and

Skills:

Understanding of terminally ill patients and families; knowledge of hospice roles; excellent assessment skills; knowledge of community resources; strong communication; documentation and program management; familiarity with managed care principles and regulatory guidelines.
