Job Title

Special Assistance In-Home (SA/IH) Program Case Manager

Job Description

The Special Assistance In-Home (SA/IH) program in North Carolina acts as a case manager for frail, elderly, or disabled adults, helping them remain safely in their homes rather than transitioning to institutional care.

Essential Duties and Tasks

Case Management & Assessment:
Conducts comprehensive assessments to determine functional status, unmet needs, and safety risks. Develops and reviews individualized service plans (Adult and Family Service Plans) at least annually.

Monitoring & Compliance:
Conducts face-to-face visits with clients at least quarterly and telephone contacts monthly. Monitors the use of SA/IH funding to ensure it addresses basic needs (rent, utilities, food).

SA/IH Administration:
Collaborates with Income Maintenance Caseworkers (IMC) to determine and verify initial and ongoing financial eligibility.

Coordination of Care:
Acts as a liaison with human service agencies, medical professionals, and family supports.

Crisis Intervention:
Responds to client or family crises, mediates conflicts, and advocates for clients within the service delivery system.

Intake & Documentation:
Documents all assessments, monitors activities monthly, and rotates with other staff to handle intake referrals.

This worker must interpret intake information accurately in order to determine if any person referred is alleged to be incapacitated, abused, neglected or exploited and in need of protective services.

Other duties as assigned by Director, Deputy Director, Program Manager or Supervisor. Disaster duty, including overnight shelter duty, is mandatory.

Instructions come from manuals for comprehension of policies and services
- State and County manuals available. Also, confidentiality laws, Code of Ethics. Methods of service provision guidelines. Worker has weekly conference and continuous open communication with supervisor, unit meetings bi-monthly and training sessions monthly, contact with other agency workers involved with common cases, frequent outside contact.

Tools and equipment used by worker are automobile, telephone, voicemail, calculator, computer, scanner and printer, files, records, state and county forms, manuals,and copier.

All forms must be precise and accurate.

Stamina and endurance required to perform job duties mentioned.

Completion of computer forms requires close visual attention.

Serious and difficult decision, difficult interviews, and completing forms require intense mental concentration.

Confidentiality to clients, informants, and in relation to information received from other agencies must be adhered to in order to safeguard clients and other persons.

Work hours are Monday through Friday, 8:00 a.m. to 5:00p.m. Worker must be available after hours in unusual, unexpected circumstances.

Skills and Abilities

The following knowledge, skills and abilities are essential to job performance. Basic knowledge of human behavior, social systems, and social institutions, awareness of social problem areas - cause and impact on individuals, families and communities - and appropriate resources and methods to work with them, basic theory and methods of social casework and groupwork; basic knowledge of reflective listening and counseling skills, theory of interpersonal communication and group dynamics.

Worker is able to recognize dysfunctional behavior, made assessments and develop basic treatment plans. Function under direct, regular supervision and act professionally on one's judgments. Have ability to establish constructive relationship with clients and significant others, have basic intervention knowledge of problem-solving techniques. Worker acts to determine client's need for service and is advised of administrative requirements. Has ability to relate as professional participant in an agency program and relates in positive or appropriate relationships under adverse conditions.

Education and Experience

Minimum Education and Experience

Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.

Special requirements:

Must possess a valid North Carolina Driver License; any restrictions on North Carolina driver's license must not prohibit the performance of the essential functions of the position.
