Performs assessment, referral, and volunteer recruitment and program documentation work for home delivered meals, frozen meals, general transportation, and medical transportation programs through the Recreation and Senior Services Department.

An employee in this class assesses, makes eligibility determinations, and makes referrals for specific meal and transportation services in the County. Work includes conducting in-home visits with clients or potential clients of these services, conducting client needs assessments with providing information and referral information addressing identified needs, documenting and recording all tasks related to each service and client interaction, managing volunteer resources assisting with recruitment, training, motivation, and retention of volunteers.

Work is performed under the supervision of the Senior Services Supervisor and is reviewed through conferences, review of reports and records, monitoring by the Area Agency on Aging, and feedback from the volunteers and citizens served.

Manages volunteer resources across the County by recruiting, training, and retaining volunteers and providing on‑going communication about the program; plans and coordinates recognition event for volunteers.

Conducts in‑home visits with clients or potential clients of senior services.

Receives referrals from other agencies, potential clients, and their caregivers; makes in‑home visits to determine client eligibility; and completes intake assessments; provides client reassessments semi‑annually for required documentation and ensures that clients who are no longer eligible cease receiving services.

Identifies client needs following through with information and referral services.

Determines client locations in relation to existing meal routes and makes new route assignments; adds and removes meal clients from routes and informs food vendors of changes.

Monitors program documentation; ensures up‑to‑date client and food vendor files; ensures volunteer route sheets and vendor information contains appropriate program documentation; ensures meals are approved and change orders documented.

Makes temperature checks on delivered food to assure compliance with program requirements; trains volunteers to perform monthly checks.

Works collaboratively with in‑house staff, as well as other agencies to ensure the most up‑to‑date information and referral information is made available.

Proactively seeks methods for performing job functions in the most efficient and thorough means possible.

Interacts with program vendors and maintains professional working relationships.

Receives continual job training to ensure an appropriate knowledge base enabling employee to perform job functions.

Contacts transportation provider to schedule trips for clients; directs service requests.

Job Duties

May make meal deliveries when volunteers are not available.

Performs related duties as required.

Considerable knowledge of federal and State requirements for aging nutrition and transportation programs, especially eligibility and food safety requirements.

Considerable knowledge of various County programs and ability to refer potential clients to appropriate agencies.

Ability to assess needs and make eligibility determinations.

Ability to motivate, plan, coordinate, organize, assign, monitor, and train a large number of volunteers.

Ability to communicate effectively in oral and written forms, including skill in public speaking.

Ability to establish and maintain proper working relationships with older adults, supervisors, and other employees.

Ability to work with and demonstrate special interest in the needs and desires of older adults.

Ability to maintain program records and documentation.

Ability to become Serv Safe certified within the first 90 days of employment.

Graduation from a college or university with an undergraduate degree in Social Work or related field and work involving care management and client assessment; or an equivalent combination of education and experience.

Special Requirements

Possession of a current NC Driver’s License.
